Your First Appointment
To book your first appointment with us, please fill out the (click on link in the left hand menu) on this page and submit it to us.
Then you can follow up with a phone call to us or await our call (which will be within 1-2 business day of your submission).
We try to accommodate acute conditions within 24-48 hrs, otherwise we will fit you in at the earliest convenient time for you and for us.
We book 1 hour to do the initial assessment and that is allocated to us and only you. Therefore it is NB to keep that preassigned time. If you are unable to keep that time we require 24 hours notice: otherwise you will be charged the full hourly rate.
The cost of the 1 hr assessment is $125.00 for most associates.
Before Your Appointment
Bring along any referral letters, scans, or reports that you have from your GP or consultant. If you don’t have them, please phone the doctor’s secretary to have them faxed to us at (416) 324-2230 or emailed to info@procarerehab.ca.
Bring along a change of clothes; that is shorts for any lower back or lower extremity problem and sleeveless shirt for any upper extremity problem (females).
Bring along running shoes if it is a lower extremity problem.
Payment for the treatment is required at the end of each session. We accept credit cards (Visa, Mastercard), and debit.
If you are looking at rebooking follow-up appointments, you may need your work schedule, blackberry etc.
Please try to arrive on time or notify us if you expect to be late. You will be charged the full amount for your appointment if you do not arrive or do not cancel it with 24 hours notice. That is because if you do not show up, we lose 2 patients, you and the person we could have fit into this slot.
